Dearborn, 7th Floor, Chicago, Illinois 60603, to CDIHC, LLC, a Delaware limited liability company with an address of 600 N. Hurstbourne Parkway, Louisville, Kentucky 40222. On June 25, 2024, a corrected registration certificate was issued in the name of the correct owner, CDIHC, LLC, but the address listed on the certificate is fictitious and incorrect. Therefore, the Registrant requests another amendment to correct the address listed on the registration certificate from 10 S. Dearborn, 7th Floor, Louisville, Kentucky 40222 to 600 N.
